Call us at (970) 262-3888 Monday through Thursday between 9:00 AM and 5:00 PM to schedule an appointment with one of our Health & Wellness Navigators.

Speak or meet with a Health Navigator to discuss your situation or ask questions. This can be done by phone, virtually or in our offices.

Step #3 - A Clear Path Forward
Focusing on your specific needs, we will set goals and determine a plan of action! By the end of the consultation you will have a clear path forward.

Your Health Navigator will follow up on your progress by contacting you and help you decide the next steps that are best for you.
